Anti-aging is a promise from 1920. Who believes that?

Anti-aging is a promise that took off in the early 1920s. Rejuvenation treatments boomed in Europe back then; today, creams are supposed to smooth wrinkles and serums to firm the skin. But most anti-aging products still target what you see as you age: the surface. Longevity asks a different question. Why does skin age at all, and what happens in the cells?
In this comparison, we take an honest look at both approaches. You’ll learn where anti-aging comes from, what a good cream can really do, and where its limits lie. Then we’ll show you what longevity means for your skin. Our position is clear: we believe healthy skin needs both worlds.

Both approaches aim to help your skin look good. The difference lies in their perspective. Classic anti-aging works on the surface, targeting wrinkles, dryness, and spots. Longevity looks at the processes beneath, such as cell renewal, energy, and repair. Researchers call these the hallmarks of aging (Lopez-Otin et al., 2023). A review by the Mayo Clinic applies this idea to the skin and calls it Skinspan. It refers to the period during which your skin stays healthy and resilient (Wyles et al., 2025).
Anti-aging and longevity aren’t mutually exclusive. The best routines use both.

The desire to look younger is an old one. In the 1920s, rejuvenation treatments boomed in Europe, including an operation developed by Viennese physiologist Eugen Steinach. Their effectiveness remained controversial (Sengoopta, 2003). Anti-aging still promises to reverse the visible effects of aging, usually with a product for a single concern. That’s what we mean by the symptom side. Find out why so many people take the wrong approach to skin aging in our main article in this series.

A good anti-aging cream can do more than many people think. Sunscreen has the strongest evidence. In a study of 903 adults in Australia, the group using sunscreen daily showed no measurable increase in skin aging after four and a half years. The skin of the others aged 24 percent more (Hughes et al., 2013). Moisturizer plumps the skin and softens the appearance of fine lines. That's why daily skincare is worthwhile.
Among retinoids, tretinoin is the most thoroughly studied, but it is a medication. Retinol and retinal are its cosmetic relatives, and research on them is growing (Milosheska and Roskar, 2022). A more recent study of 56 women offers promising evidence that stabilized retinal may improve skin density and elasticity over 24 weeks, although there was no comparison group (Deda et al., 2026).
Longevity literally means a long life. But in research, the focus is less on living as long as possible and more on healthy years. In 2013, researchers described nine hallmarks of aging; in 2023, they expanded the list to twelve (Lopez-Otin et al., 2013 and 2023). These include damage to genetic material, declining cellular repair, tired mitochondria—the cell's power plants—and silent inflammation. In the skin, these processes lead to reduced elasticity, a thinner epidermis, and slower renewal (Wyles et al., 2025).
One example is NAD+. Every cell needs this molecule for energy and repair. NAD+ levels decline in many tissues with age (Covarrubias et al., 2021). NAD+ in skincare is one of the most exciting fields of research. Initial tests on skin cells in the lab are promising (Kang et al., 2024). Our article explains what we know so far about the effects of NAD+.
Skin longevity means treating your skin so it stays healthy for a long time. The Mayo Clinic overview recommends practical measures. These include lifelong sun protection, a balanced diet, regular exercise, and moisturizing skincare. UV light, smoking, and air pollution, on the other hand, accelerate aging (Wyles et al., 2025). Getting enough protein is also part of a good foundation. Find out more in our article about protein and longevity.
Anti-aging in the traditional sense means treating visible signs deliberately. That’s not wrong—in fact, many anti-aging products do exactly what they’re supposed to: nourish, protect, and smooth. The problem comes when a product promises more than it can deliver. Also take a look at the ingredients. Some substances in cosmetics are controversial.
Advantages:
Disadvantages:
Longevity takes a years-not-weeks approach and asks how your skin can retain its ability to renew itself. This makes the approach more comprehensive than anti-aging. Many new active ingredients are currently being researched, but the fundamentals are already solid today. Based on current knowledge, getting enough sleep, staying active, eating a balanced diet, and protecting your skin from the sun provide the best foundation. And that’s good anti-aging, too.

Anti-aging or longevity in brief
Anti-aging skincare addresses visible signs like wrinkles
Longevity looks at the causes within the cell
Retinal and NAD+ bring both worlds together
We recommend a routine that combines both
Anti-aging skincare addresses visible signs like wrinkles and spots. Longevity asks why cells age and aims to keep skin healthy and resilient for longer.
Sun protection is worthwhile as an anti-aging foundation at any age. Introduce retinoids gradually, and if in doubt, ask your dermatologist whether they're right for you.
It hydrates and softens the appearance of fine lines. With sun protection, it slows aging caused by UV light, but deep wrinkles remain.
The goal is to help skin retain its ability to renew itself for as long as possible. Lifestyle, sun protection, and skincare all play a role.
Retinal is a retinoid and therefore part of classic anti-aging skincare. It stimulates skin renewal and is introduced gradually.
Skin longevity is about taking a long-term approach to skincare. Sun protection, hydration, sleep, exercise, and a balanced diet are the foundation.
Daily sun protection is the most important anti-aging step. In a study spanning four and a half years, skin visibly aged more slowly with daily sun protection.
